Male Infertility and Impotence

Infertility affects up to 12% of all males and sexual dysfunction occurs frequently in men of reproductive age which causing infertility in some instances. Infertility men can experience a heavy psychological burden. The contribution of the male factor alone to couple infertility is believed to be about 20%.

Infertility is the failure to conceive within one or more years of regular unprotected coitus. Healthy spermatozoa should undergo with necessary changes for high deposition and good sperm motility are the major male factors essential for conception.

Causes of male infertility can be attributed to:

  • Sperm abnormality
  1. Oligospermia (sperm count is less than 20 million per mL)
  2. Azoospermia (no spermatozoon in the semen)
  3. (Asthenozoospermia) Loss of motility
  4. Teratozoospermia (abnormal sperm morphology – round headed sperms
  5. Aspermia (failure of emission of semen)
  6. Necrozoospermia (dead or motionless spermatozoa)
  • Erectile dysfunction
  • Ejaculatory defect – Premature, retrograde or absence of ejaculation
  • Errors in seminal fluid – High or low volume of ejaculate, low fructose content, high prostaglandin content and undue viscosity.
  • Defective spermatogenesis – The formation of sperms is known as spermatogenesis. This requires an optimum temperature 1-2 degrees lower than the normal body.
  • Congenital – Birth defects like undescended testes, cause depressed spermatogenesis.
  • Hypospadias – A congenital defect where the opening of the penis is on the underside rather than the tip. This causes failure to deposit sperm high in vagina.
  • Autosomal (chromosomal) diseases like Kartagener Syndrome – Loss of ciliary function (mobility) and sperm motility.

Risk factors for male infertility:

  • Chronic illnesses affect for healthy sperm production – Diabetes, gonorrhoea, mumps or HIV, chlamydia like infections, varicocele (enlarged veins in the testes).
  • Environment factors – radiation, periodic exposure to heat, pesticides, endocrine disrupting chemicals (EDCs).
  • Injuries – cause sperm delivery problems and blockages when damaging to the genitals area.
  • Lifestyle choices– Cigarette Smoking can have adverse effects on DNA in sperm and heavy drinking can affect the quality of sperm with erectile dysfunction.

Semen analysis is the most important test which could rule out male infertility. In-depth analysis of hormones, fructose content, microscopic studies may also be performed when advised by the doctor.
